Background: 

Phosphodiesterase, or PDE, is an enzyme which hydrolyzes phosphodiesterase bonds. PDEs regulate subcellular signal transduction through the degradation of cyclic nucleotides. Inhibition of PDEs has become an exciting area of clinical significance. Through inhibiting PDEs, cyclic nucleotides like cAMP and cGMP can remain active and act as second messengers. PDE inhibition can have widespread pharmacological effects depending upon the type of phosphodiesterase that is inhibited.  

Through inhibiting phosphodiesterase 3, intracellular concentrations of cAMP rise. In combination with other intracellular messengers, cAMP stimulates cardiac muscles. Many PDE3 inhibitors have been developed to treat cardiac failures, cardiogenic shock, and hypertension. Since heart disease is the leading cause of death in the United States, PDE inhibition has become a major pharmacological target of interest.
